Targeted Case Management

Case Management connects individuals with mental health and behavioral concerns to the community services they need. Each individual who is referred and authorized to receive services is assigned a Case Manager to coordinate their care. Following a thorough assessment, the Case Manager and the individual will work together to develop a treatment plan that identifies and prioritizes their service needs. The Case Manager will then connect the individual to the appropriate service provider. A successful outcome is achieved when the individual obtains services and supports needed to live independently in the community and no longer requires case management services.

Case Management Services

  • Targeted Case Management - Provides assistance to individuals with chronic mental illness in accessing community services and supports.
  • Forensic Case Management - Provides specialized care for individuals involved in the criminal justice system. While incarcerated, the individual is assessed in all areas of daily living. Upon release, Case Management services provides the level of assistance required to help the individual maintain mental health treatment and reduce recidivism.
  • Child and Adolescent Case Management – Provides assistance to children with mental illness in accessing resources and supports in the home and community.  Alliance Case Management also partners with Harford County Schools in providing School Based Mental Health services.

Alliance provides Case Management Services to Harford, Howard and Baltimore County residents in need of Mental Health Services. The Case Management Division of Alliance, Inc. is accredited/certified by:

  • Department of Health and Mental Hygiene(DHMH)
  • Commission of Accreditation of Rehabilitation Facilities(CARF)
